Perth, Australia in the year 2000. A health and safety instructor was making a video on the ins and outs of driving a forklift safely. During the filming he managed to be thrown from the cab of the vehicle and crushed to death. After a thorough investigation it was found that he had been going too fast over varied terrain and had not been wearing a seat belt. He’s probably glad he isn’t around to receive the ribbing that his Aussie mates would have no doubt dished out.
